Investigation Reveals Train Safety System Failure in Fatal Mid-Wales Crash
An investigative report has revealed that a blocked wheel-slide safety system contributed to the recent train crash near Llanbrynmair, Wales, which resulted in the death of 66-year-old David Tudor Evans and serious injuries to four others. The Rail Accidents and Investigations Branch found that the automated sanding system, designed to enhance braking by increasing wheel traction, was unable to function due to blocked hoses. The westbound train failed to stop at a loop crossing, ultimately colliding with an eastbound train. Ongoing investigations are assessing the collision speed and related factors.
